HC Connect Patient Centric Portal offers an online Personal Health Records service to patients and providers, We take the responsibility of manageing the technology routines that will enable the automatic transfer of patients’ health records from providers’ EHR systems to our portal and vice versa.. Our Portal typically seeks participation from both providers and consumers. A provider participates by uploading its patients’ records to HC Connect. Once records are on our portal, the consumer can gain electronic access to her records. Having HC Connect account reduces the cost a patient incurs when transferring health records to a new provider. All the patient needs to do is to either grant appropriate access rights to the new provider (if that provider is on the HC Connect) or download the records and submit them either electronically or physically (if that provider is not on HC Connect).

In the health care provider market, if a patient has established a provider-patient relationship that involves a complete medical history stored by that provider, and thus faces "switching costs" when changing providers. These costs can include re-establishing a medical history through requesting copies of records from the previous provider and delivering them to the new provider, filling out forms at the new provider’s office, as well as repeating certain tests in case previous test results are missing, incomplete, or simply deemed uninformative or unacceptable by the new provider.
